Interactive Discussion Guide
Get help telling your story to your doctor. The more clearly you communicate your symptoms and treatment history, the more likely your doctor will be able to recommend the right treatment for your chronic pain.
Our three-step interactive discussion guide will help you document the critical information your medical team needs to assess and treat your chronic pain. You can complete them in any order or just use the ones that are helpful for you.
Step 1: Map Your Pain
Create a map of your body that describes where you have pain, what kind of pain you feel, and the severity and frequency of your pain.
Step 2: Pain & Treatment History
Fill out a short questionnaire to track the doctors you've seen, how long you've had pain, and what treatments you've tried.
Step 3: Pain Journal
Use this journal to track your pain levels, making notes of what you were doing that seemed to make your pain better or worse.
