Our three-step interactive discussion guide will help you document the critical information your medical team needs to assess and treat your chronic pain. You can complete them in any order or just use the ones that are helpful for you.


Step 1: Map Your Pain

Create a map of your body that describes where you have pain, what kind of pain you feel, and the severity and frequency of your pain.

Step 2: Pain & Treatment History

Fill out a short questionnaire to track the doctors you've seen, how long you've had pain, and what treatments you've tried.

Step 3: Pain Journal

Use this journal to track your pain levels, making notes of what you were doing that seemed to make your pain better or worse.
