Medtronic Patient ID Card
Your patient ID card contains important information about your therapy and includes your physician's phone number in case of an emergency. It's important to keep the card with you at all times because:
- It identifies you as having a Medtronic pain therapy in case of an emergency.
- You should show your card when you have any medical or dental procedure or when you must pass through a security screening system.
Getting your ID card
You received a temporary ID card when you got your Medtronic pain therapy. A permanent card will be sent to you within three or four weeks. If you do not receive your permanent card or you misplace it, call us at (800) 551-5544.
When you get your permanent card, make sure the information is correct. You can make changes to your information on www.medtronic.com.
